Serenity Academy of Performing Arts, LLC & Serenity Event Center, LLC Liability Waiver

I understand that there are risks of physical injury associated with, arising out of and inherent to the activity of performing arts and party activities. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all right and/ or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to Serenity Academy of Performing Arts, LLC & Serenity Event Center, LLC, it’s officers, agents, employees, instructors, subsidiaries, parent corporations, and all affiliated entities.

I hereby agree to release Serenity Academy of Performing Arts, LLC & Serenity Event Center, LLC, and hold Serenity Academy of Performing Arts, LLC & Serenity Event Center, LLC, harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation in performing arts and party activities on behalf of the participant. I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the risk of injury. I have signed this document voluntarily and of my own free will in exchange for the privilege of participation. I also give Serenity Academy of Performing Arts, LLC & Serenity Event Center, LLC, permission to use my child’s picture in or on any form of advertisement for Serenity Academy of Performing Arts, LLC & Serenity Event Center, LLC, or a Serenity Academy of Performing Arts, LLC & Serenity Event Center, LLC, affiliated event.

If I am a minor, my parents and / or legal guardian has also signed this document releasing Serenity Academy of Performing Arts, LLC & Serenity Event Center, LLC, from any and all such liability described above and has acknowledged that I am knowingly and voluntarily assuming all risks of injury inherent to this activity. The participant has my permission to participate in Serenity Academy of Performing Arts, LLC & Serenity Event Center, LLC, Events. I warrant this information is complete and correct. By purchasing tickets, I further release Serenity Academy of Performing Arts, LLC & Serenity Event Center, LLC, of all liabilities associated with my child’s attendance at Serenity Academy of Performing Arts, LLC, & Serenity Event Center, LLC. I agree that if my child displays any violent, lewd, or rude behavior, they will be asked to leave with no refund due to me. I agree that alternate transportation will be arranged if I or anyone sent to pick my children up appear to be under the influence of alcohol or drugs and not safely able to operate a vehicle. I am agreeing that I have reviewed the above info and have had the opportunity to ask questions prior to participation and before purchasing my ticket.
